THE PROGRAM:

Evergreen World Adult Day Health Care (ADHC) is a community-based program, inclusive of Nursing, Social Services, Licensed Clinical Social Worker, Physical Therapy, Occupational Therapy, Speech Therapy, Nutrition, and Activities. Evergreen World offers health support based on members’ medical needs and assistance of daily living.

THE GOALS:

Evergreen World ADHC welcomes all adults to Optimize Quality of Life. Our program provides members daily Health monitoring, Social support counseling and Skilled physical exercises in conjunction with social bonding enjoyment, musical talent participation and fun recreational activities. These services are conducted by our professional Multi-Disciplinary Team to maintain members’ Optimal Health.

Evergreen World ADHC symbolizes a rejuvenating atmosphere with a safe, harmonious, happy, and friendly “evergreen” environment. These goals are restorative measures to prevent premature hospitalization.
